Shen Hongbing said that one direction in the future is to dynamically adjust and optimize the national immunization program strategy, and optimize it according to the principles of overall planning, dynamic adjustment, and step-by-step implementation. "We are preparing to make a five-year plan," he said. The future goal is to gradually narrow the gap between the domestic immunization program and the international level, and gradually include vaccines that need to be included.
The last time China substantially included a new vaccine was in 2007. The adjustments in recent years mainly involve vaccination procedures and strategies. Therefore, there is a gap between the number of vaccines in China's national immunization program and the international level. The PPT presented by Shen Hongbing shows that WHO recommends that all countries include 10 vaccines in the immunization program, and China has four vaccines that are not included (Hib, PCV, HPV, and rotavirus vaccines). Developed countries such as the United Kingdom and the United States generally use combined vaccines (such as quadruple, pentavalent, and hexavalent) and better processes (such as polysaccharide conjugate vaccines), while my country mainly uses single vaccines and the process needs to be upgraded. The immunization program vaccines in countries such as the United Kingdom and the United States cover a wide range of people, including adolescents, adults, and some special populations, and the types of vaccines are more abundant, while the immunization program vaccines in my country mainly cover children under 6 years old, and lack vaccines suitable for adults, especially pregnant women, the elderly and other key populations.
He also mentioned that the cost of treating diseases caused by infections such as HPV, Hib, varicella virus, and pneumococcus in China far exceeds the cost of vaccination.
Shen Hongbing explained that there are many factors affecting the adjustment of the immunization program, including disease prevention and control needs, disease burden, vaccine characteristics, economic benefits, vaccination conditions, mass needs, financial financing capabilities, and connection with the existing planning system. The research directions for the dynamic adjustment of the national immunization program in the future include:
First, add new types of vaccines. Those with international consensus, heavy disease burden, high cost-effectiveness, vaccine production capacity that meets demand, and financial affordability should be added as soon as possible.
Second, optimize the existing vaccine immunization program. Optimize according to epidemic characteristics, changes in prevention and control goals, vaccine characteristics, etc.
Third, explore the vaccine withdrawal procedure. Vaccines that are not universally vaccinated for the entire population, and existing vaccines that are not suitable for disease prevention and control needs should also be considered for vaccine withdrawal or converted to reserve vaccines. He mentioned that the National Center for Disease Control and Prevention received relevant suggestions for thinking about the withdrawal procedure during discussions with the Ministry of Finance.
Shen Hongbing said that in 1974, the World Health Assembly recommended that countries expand their immunization programs. The essence of the national immunization program is to ensure that all recipients have fair vaccination opportunities to protect their health. According to the characteristics of the epidemic and the burden of the disease, vaccine accessibility and affordability, determine the types of vaccines and target populations of the national immunization program. Over the past 50 years, 154 million lives have been saved worldwide.
