When applying for a Medical Device Registration Certificate, you should not start by simply asking "what documents to prepare". The more accurate workflow is to first determine the classification of the medical device, confirm the competent regulatory authority for acceptance, and finally prepare the testing, clinical evaluation, quality system and registration application documents.
In China, Class I medical devices are subject to filing management, while Class II and Class III medical devices are subject to registration management. That is to say, the "Medical Device Registration Certificate" in the general sense mainly corresponds to Class II and Class III medical devices. Domestic Class III medical devices, as well as imported Class II and Class III medical devices, are reviewed and approved by the National Medical Products Administration (NMPA); domestic Class II medical devices are generally the responsibility of the provincial drug regulatory authorities.
Before initiating the process, the first priority for enterprises is to complete product classification judgment. Factors including the product's intended use, structural composition, working principle, whether it is active, sterile, implanted in the human body, or used for diagnosis or treatment will all affect the final classification result. If the classification judgment is inaccurate, subsequent registration paths, testing items, clinical evaluation requirements and application documents will all go wrong accordingly. The slow progress of many enterprises in registration does not stem from their inability to write documents, but from the failure to clarify the product boundary and management category in the early stage.
After the classification is confirmed, it is necessary to clarify the application entity and production relationship. The registration applicant shall be responsible for the safety, effectiveness and quality controllability of the product. For domestic products, the relationship between the registration applicant, the manufacturing enterprise and the entrusted production party shall be straightened out; for imported products, the overseas registration applicant shall designate an enterprise legal person within the territory of China to submit the registration documents, and provide the marketing authorization document issued by the competent authority of the country or region where the product is originally manufactured.
Then it enters the technical document preparation stage. Enterprises need to complete the product technical requirements, product instructions and label drafts, summary documents, research documents, risk analysis documents, manufacturing information, clinical evaluation documents, conformity statement and other contents. The NMPA has promulgated the Requirements for Medical Device Registration Application Documents and the formats of approval documents, which came into effect on January 1, 2022. The application documents cannot be organized arbitrarily by enterprises, but must be prepared in accordance with the corresponding catalogue and formats.
Among all these documents, the product technical requirements are of great importance. It is not promotional material, but a critical basis for subsequent testing, technical review and regulatory supervision of registered products. It usually specifies the objectively verifiable functional and safety indicators of the product, as well as corresponding testing methods. Ambiguous product technical requirements will adversely affect subsequent registration testing, technical review and change management.
During the document preparation process, registration testing shall also be completed. Enterprises can choose a qualified testing institution for inspection; under eligible circumstances, they can also submit self-inspection reports in accordance with regulations. Testing is not simply generating a report, but must be consistent with the product technical requirements, applicable standards, product models and specifications. If the models, structural composition and key performance indicators are not confirmed before the product is sent for testing, problems such as supplementary testing, re-testing or inconsistent documents are very likely to occur afterwards.
Clinical evaluation is also a link that easily causes delays in the registration process. Not all medical devices require clinical trials. Instead, enterprises need to judge whether they can complete clinical evaluation through equivalence comparison with reference products, clinical literature, clinical data or other paths according to product characteristics. For products that need to carry out clinical trials, the requirements for clinical trial management shall be followed; some high-risk Class III medical device clinical trials also require clinical trial approval.
Meanwhile, enterprises shall establish and maintain a quality management system compatible with their products. The regulatory authority may carry out quality management system verification through document review or on-site inspection during the registration process. For enterprises, the registration certificate cannot be obtained simply by a set of application documents. The whole process including R&D, procurement, production, testing, release, traceability, change management, adverse event handling and post-marketing supervision must be able to support the controllability of product quality.
After the technical documents, test reports, clinical evaluation and system documents are fully prepared, enterprises can submit the registration application through the corresponding government service channels. The regulatory authority will first conduct a formal review. Applications with complete documents that meet the requirements will be accepted; applications with incomplete documents or not in statutory forms will require document supplementation. After acceptance, the application will enter the technical review stage, during which the review team may continue to request supplementary materials. The technical review time limit for Class II medical device registration applications is generally 60 days, and that for Class III medical device registration applications is generally 90 days. The technical review time limit after supplementary document submission will be calculated separately.
After the technical review is passed, the regulatory authority will make an approval decision. For applications that meet the requirements, the Medical Device Registration Certificate will be issued; for those that do not meet the requirements, the registration will be rejected with a clear explanation of reasons. After obtaining the registration certificate, enterprises must fulfill subsequent responsibilities including renewal registration, change registration or filing, post-marketing supervision, and adverse event monitoring. When there are substantial changes to the product design, raw materials, production process, intended scope, usage method, etc. that may affect product safety and effectiveness, enterprises need to apply for change registration; other changes shall be filed or reported in accordance with relevant regulations.
Therefore, the application for a Medical Device Registration Certificate can be roughly summarized as the following path: confirm the product classification and registration authority first, clarify the application entity and production relationship, then prepare product technical requirements, research documents, instructions and labels, risk management, test reports, clinical evaluation and quality system documents. After submitting the registration application, you will go through acceptance, technical review, document supplementation and approval, and finally obtain the registration certificate.
What enterprises should prioritize in the early stage is not "gathering all documents together", but sorting out the product classification, technical requirements, testing path, clinical evaluation path and quality system foundation. Clarifying these points in advance will effectively avoid repeated document supplementation in subsequent application processes.
